In Super Mario World, it is possible to carry an item while in a P-Balloon state, by grabbing it and the P-Balloon on the same frame. The item will stick to the side of Mario it was in before being grabbed until the power runs out.

Top: the boss Manhandla from the NES The Legend of Zelda is described in the North American manual as a “large man-eating flower”.
Middle: however, this omits a Super Mario Bros. reference from the original Japanese version of the manual, which calls it (translated) “a four-limbed, jumbo-sized Piranha Plant”. The words “Piranha Plant” are marked in yellow.

It is unknown why the North American manual dropped the reference; possibly, the translator was not familiar with Super Mario Bros. and simply did not recognize it.

Japan-only add-on for the Super Famicom that allowed it to receive
satellite transmissions; while it is most commonly known for featuring
games, it also offered shows that had live audio alongside a slideshow
of pictures.
In Super Mario 64 DS, if Mario picks up a Luigi or Wario hat on the same frame as touching a Power Flower, his head will disappear on that frame (the issue can also be observed on original hardware; however, the internal resolution of the game has been increased for this screenshot to provide a clearer view).
In the Sand Spiral Galaxy in Super Mario Galaxy, a red Toad can be encountered. The only Toads that are seen in Super Mario Galaxy outside of the Peach’s Castle area are the Toad Brigade, which explicitly follow Mario around in their Starshroom spaceships. Since the Toad Brigade includes only one red Toad, being Captain Toad, this Toad is likely meant to be Captain Toad, appearing here without his backpack and headlamp due to some developer oversight. If this is the case, then this is the only instance where Captain Toad has been seen without his gear in any official media.
