Top: the cave background with the windows and glowing berries is used several times throughout Yoshi’s Island.
Middle: However, usually, only part of it is loaded; the full background is only loaded in a single room in the game, in Level 2-8. However, even at the highest point in the room that Yoshi can reach, the top of the background is not visible; it appears as though the background simply continues as darkness with occasional blue pixels.
Bottom: however, that is not the case. Using a map viewer, we can see that the top of the background reveals that the darkness is actually a forest, with silhouettes of trees becoming visible. This means that what appears to be a cave background has actually been a forest background all along, though it does not explain the bizarre existence of windows in that forest.

Unique room found in development files for Yoshi’s Island that uses graphics completely absent from the rest of the game, including doors that appear to be taken from the Kirby series. The room is designed to be able to be rotated, so that the changing gravity would be used to reach the other door, which is oriented upside-down relative to the first door.

In Yoshi’s Island, letting Yoshi be hit by the tall Stretch Shy Guys may result in the Shy Guy becoming stuck in a glitched state where he will very frantically, yet very slowly, keep backing away from Yoshi.

There are unseen upside-down smiling clouds in Bowser’s Castle in Yoshi’s Island.
Top: the final level of Yoshi’s Island, Level 6-8, is Bowser’s Castle. The first room of the level takes place outside the castle. In the background, hills and the moon can be seen. The screen does not scroll vertically, making it impossible to see what is in the background above the moon.
Middle: extracting the background image from the game’s data, we can see there is an unseen area with clouds above the moon (the top of which can be seen cut off on the bottom).
Bottom: looking closely at the clouds, it becomes apparent that they actually have upside-down smiling faces (left: as they appear in the background, right: rotated 180 degrees). These also appear in other backgrounds in the game, though being oriented correctly. It is unknown if this is some sort of bizarre oversight or if putting the clouds upside-down was intended to be a visual indication of the evilness of the location.

In Yoshi’s Island, some rooms in castle levels have randomly generated wall tiles, which make the room appear very subtly different every time it is entered. Here are several screenshots of entering a single room several times, played in a sequence.
